She chose to keep her maiden name "Willi" because she's really made a big name for herself over the years in her practice of mesothelioma surgery. She's pioneered several breakthrough surgical methods that have already started to reduce the need for radiation and chemotherapy as a post-operative treatment. Given the great toxicity levels inherent in these procedures, as well as the ubiquity of their practice, this practically makes her a goddess in this town.

As for the "Chilli" in her name, she's Catholic, and at her confirmation at age 13 she chose Chilli as her confirmation name. Frankly, she did that just so her name would rhyme, but it posed a big problem in the eyes of the Church, because they did not recognize any Saint Chilli.
